Trail Report Form

To report an accident, suspected illegal activity or maintenance problem on any segment of the Ice Age Trail, please download, complete and submit a Trail Incident Form to the Wisconsin DNR's State Trails Coordinator.

For accidents and suspected illegal activity, contact local law enforcement officials as soon as possible after the incident, in addition to submitting an incident form.

For maintenance issues, please notify the Ice Age Trail Alliance (800-227-0046, info@iceagetrail.org) in addition to submitting an incident form so that we can add your comments to the Trail Update portions of our Explore the Trail pages.
